MEDICAL CONSENT/RELEASE FORMS Youth Ministry requires current medical consent/release forms to be on file in our office for every person participating in youth group . They are good for one year and kept on file from August 1, through July 31, in the youth ministry office. Please complete a form for your child and return it to Mrs.Testa at the rectory. Thank you.
